How does LiveBig work with support coordinators?

We have a good understanding of how critical the role of support coordinators is to NDIS participants based on our combined years of experience in the disability sector. LiveBig is committed to respond to enquiries, concerns, requests, and referrals for services most relevant to their participants. You express your needs and we will draft a service agreement for you outlining our proposed services to meet your needs.

Sign Your Service Agreement

We will explain the Service Agreement to you. Once you’re happy with the agreement, sign and send it back.

Find The Right Match

We will allocate you a clinician who meets your preferences and needs.

Schedule Your First Appointment

Your clinician will contact you to arrange your initial appointment.

Begin Your Therapy Plan

After your initial appointment, we will work with you to develop a therapy plan to start your journey toward your goals.

Service Delivery

We will provide services that meet your needs (e.g. reports, assistive technology, home modifications, intervention programs).

Review Progress

We will review progress regularly and seek your input on changes to the therapy plan. When your plan is due for review, we will provide you with a report.

Service Renewal

When your new plan is ready, we’ll develop a new service agreement.

 

What Can I Expect When I Start with LiveBig

Enquiry Stage

At this stage, Maria will speak to Support Coordinators about LiveBig’s service capacity, service coverage and any preference a Support Coordinators’ participants would have before sending a full referral. Once we identify that we have service capacity in a participants’ location and preferences can potentially be considered, Support Coordinators will be able to send through a full referral.

Referral Received Stage

On reception of the full referral, LiveBig will start to match suitable clinicians to support the participant. This matching is based on a list of criteria including location, suitable experience and other factors that consider a clinician’s area of specialty.

Contracting Active Stage

Our CSC (Corporate Service Coordinator) will issue a Service Agreement and Consent Form to authorised signatories to review and sign. We furnish a copy of the Service Agreement and Consent form to Support Coordinators so they can also review and keep them in the loop to be sure that these documents are signed and returned on a timely manner. Once we receive the signed documents (Service Agreements and Consent Form), the CSC Team will provide an update to the participant/family and Support Coordinators about the next steps.

Allocation Stage

Once the clinician is assigned, the CSC team will update the participant/family/Support Coordinators accordingly via email and the initial appointment organised by the assigned clinician.
